I've only been able to get a working install of Server 2012 once, then during some updates it froze and the vhd appeared to lose its reference and I had to delete the whole thing and start over. Ever since then, every time I try to reinstall I get a driver irql_not_less_or_equal after the setup goes through OOBE. I've deleted all tmp files, re-uploaded the ISO, regenerated the qcow2 virtual disk, tried in a new lab, rebooted eve etc.

Nothing seems to be able to fix it.

I too am having this issue, but this is with Windows 10 and Windows 2016 Server. I am investigating the use of new drivers. This is the link I have found on this issue:
