I have my primary account. When I create a second account, the webpage hung. After a while, I went back to the account management page. Just the original account. I tried to create the second account again, it complains of "database error". I can't login as the new user.
Is there a way to reset that user and try again? Or is this a known issue?

Yes you had a bit screwed your EVE system, but it is solvable
